Claim Form Instructions
This claim form is for use in submitting a claim for a cash payment in the Settlement between YouTube TV Plaintiffs, DirecTV Stream Plaintiffs, and The Walt Disney Company in Heather Biddle, et al. v. The Walt Disney Company, No. 5:22-cv-07317-EJD. Further information about the lawsuit and additional copies of this claim form are available here .
The Settlement Classes consist of the YouTube TV Settlement Class and the DirecTV Stream Settlement Class.
The “YouTube TV Settlement Class” consists of all Persons who purchased a YouTube TV subscription from the period beginning April 1, 2019, through March 31, 2026.
The “DirecTV Stream Settlement Class” consists of all Persons who purchased a DirecTV streaming live pay TV subscription (branded at various times as, at least, DirecTV Stream, DirecTV Now, and AT&T TV Now) from the period beginning April 1, 2019, through March 31, 2026.
The easiest way to submit a claim is online, or you can print out, complete, and mail the entire claim form to the mailing address listed on the claim form. Claims must be submitted online or mailed by September 8, 2026.
How to File Online:
Before Claim Filing: You will need the Unique ID printed on the Notice that you received in the mail or email. If you did not receive a Notice or have lost yours, please contact the Settlement Administrator at info@OnlineTVSettlement.com for assistance.
During Claim Filing: Please be sure to complete the entire claim form and provide the requested information so that the administrator can validate your claim form. The administrator may contact you for additional information if necessary and may use data provided by YouTube TV and/or DirecTV Stream to validate your claim.
After Claim Filing: After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to it if you have any questions about your claim form.

Remember, all claim forms must be submitted online or received by the Settlement Administrator no later than September 8, 2026 .
If you received an email or postcard notice indicating that you are part of the Settlement Class, you will have a 10-character alphanumeric "Unique ID" and a 4-digit "PIN". Both values will be found on your email or postcard notice.
